APPLICATION FOR FMSA TRAVEL GRANT

1.  The FMSA travel grant is for active members and associate members in a total claimable sum of RM3000 to present at overseas conference relevant to Family Medicine and Pri-mary Care who has not successfully claimed this grant in the previous 5 years.

2. To apply, kindly provide the following at least 1 month before conference:

• Application letter addressed to the President
• Abstract acceptance from conference organizer
• Conference poster & scientific programme

3. For papers & presentation related to MOH, DG approval for presentation/publication.
4. To claim once approved, kindly provide the following latest by 1 month after confer-ence:

• FMSA Exco approval letter
• Completed claim form
• Original receipts
  A picture of yourself presenting at the conference (soft copy)
• A report (soft copy)

5. For members who are under government service (MOH, MOE and MOD), approval to travel overseas is required according to the general orders and expected to be ob-tained individually according to the stipulated duration prior travel.

APPLICATION FOR FMSC SPECIAL GRANT

The FMSC special grant is for active members and associate members in a total
claimable sum of RM1000 to present at the FMSC.

To apply, kindly provide the following at least 1 month before conference:
1. Application letter addressed to the President
2. Abstract acceptance from conference organizer
3. Conference poster/ oral & scientific programme
4. For papers & presentation- acknowledgement to FMSA in the presentation

To claim once approved, kindly provide the following latest by 1 month after conference:
1. FMSA Exco approval letter
2. Completed claim form
3. Original receipts
4. A picture of yourself presenting at the conference (soft copy)
5. A report (soft copy)

Report and pictures submitted may be used by FMSA as material for FMSA
newsletter and/or be uploaded onto FMSA social media platforms and official
website.

Who can apply?
• The principal investigator needs to be an active FMSA member
• The project must be relevant to the primary care practices

How to apply?
• Applicants need to submit all the necessary documents to the President and Research Committee member of FMSA. The requested documents are:

Research application form
Curriculum vitae of the investigators involved
A copy of the Good Clinical Practice (GCP) Certificate
A complete proposal that covers: Title, list of investigators, study background or intro duction, ob jectives, methodology – study design, sample population, data collection, plan of data analysis and
Gantt chart
Propose budget
• The maximum total allocation is RM10,000 per year and the deadline for application is 31st May. However, the sum for each application will depend on the number of applica-tions in the current year.

What are the roles of the investigator(s)?
1. T o register the project at National Medical Research Registry (NMRR)
2. To claim for any expenses approved by FMSA committee with proof of payments (receipts)
3. To report to the FMSA every six months on the progress of the project
4. To write a full report and send it to FMSA research exco once the project is completed for documentation claims
• To share the findings of the research to FMSA members and/public preferably through a publication at a refereed journal or presentation at any conferences including FMSA Sci-entific Conference
How do you know your application is approved?
• The Research Committee will give feedback to you within eight weeks of your applica-tion.

TERMA DAN SYARAT – SYARAT PERMOHONAN PERUNTUKAN LAWATAN SAMBIL BELAJAR ‘STUDY TOUR’

1. Lawatan sambil belajar secara kumpulan perlu berkaitan dengan kesihatan primer
2. Semua peserta lawatan adalah terdiri daripada pakar perubatan keluarga
3. Setiap kumpulan perlu dianggotai oleh sekurang-kurangnya 5 orang ahli
4. Semua pakar perubatan keluarga yang terlibat dalam lawatan adalah ahli FMSA yang aktif pada tahun semasa
5. Kertas kerja lawatan perlu dihantar 1bulan sebelum tarikh projek
6. Pemohon kertas kerja lawatan hanya dibenarkan untuk membuat permohonan setiap 3 tahun.
7. Namun tiada halangan untuk pemohon atau peserta lain (yang bukan pemohon kertas kerja lawatan pada tahun semasa) untuk menyertai lawatan yang dianjurkan pada tahun berikut dan seterusnya
8. Lawatan hanya boleh dilakukan setelah mendapat kelulusan secara bertulis daripada FMSA
9. Laporan selepas program perlu diserahkan dalam masa sebulan
10. Jawatankuasa kumpulan diminta mengemukakan maklumbalas kepada FMSA sekiranya bersetuju dengan terma dan syarat seperti diatas
11. Sebarang tuntutan kewangan hendaklah dibuat dengan Bendahari Kehormat bersama tuntutan resit-resit rasmi

TERMA DAN SYARAT – SYARAT PERMOHONAN PERUNTUKAN [RM 3,000/ NEGERI] PRO-GRAM PERKHIDMATAN PRIMER ‘PRIMER PROJECT’

1.  Project perlu melibatkan anggota kesihatan dan berkaitan kesihatan primer.
2. Semua pakar perubatan keluarga yang terlibat adalah ahli FMSA yang aktif pada tahun semasa
3. Kertas kerja perlu dihantar 3 bulan sebelum tarikh projek
4. Seorang wakil exco FMSA perlu hadir bersama pada hari program dengan peruntukan RM 500 (ditanggung oleh FMSA) dan tertakluk dengan syarat-syarat tertentu.
5. Ucapan penghargaan kepada FMSA di dalam ucapan perasmian dan buku program
6. Menggalakkan penglibatan FMS dari swasta atau university
7. Laporan selepas program perlu diserahkan dalam masa sebulan
8. Jawatankuasa program diminta mengemukakan maklumbalas bertulis kepada FMSA sekiranya bersetuju dengan terma dan syarat seperti diatas
9. Sebarang tuntutan kewangan hendaklah dibuat dengan Bendahari Kehormat bersama tuntutan resit-resit rasmi
10. Semua negeri dan 1 universiti layak untuk memohon peruntukan di bawah projek pri-mer setiap tahun

PROJEK KOMUNITI BERSAMA PERSATUAN PAKAR PERUBATAN KELUARGA(FMSA) MALAYSIA

Terma dan syarat :

1. Projek perlu melibatkan komuniti dan berkaitan kesihatan primer.
2. Semua pakar perubatan keluarga terlibat adalah ahli yang aktif pada tahun semasa.
3. Kertas kerja perlu dihantar 3 bulan sebelum tarikh projek.
4. Seorang wakil exco fmsa perlu hadir bersama pada hari program dengan peruntukan RM
   500 (di tanggung oleh fmsa) tertakluk dengan syarat-syarat tertentu.
5. Ucapan penghargaan kepada fmsa di dalam ucapan perasmian dan buku program
6. Menggalakan penglibatan fms dari swasta atau university memberi sijil penghargaan yang dibingkaikan
7. Laporan selepas program perlu di serahkan dalam masa sebulan bersama tuntutan resit
